New Kit Registration

Registering Your OneTest Kit

Please take a look at our Portal Registration Guide before you get started. Please complete the kit registration process only when you have received your kit or requisition form.

Before you submit your sample, you will need to register your OneTest kit to a OneTest account. 

Please note: All patients MUST have their own individual account, there are no shared accounts for OneTest.

Anyone with a valid email address can create a OneTest account at any time. Although you enter an email address when placing an order in the OneTest store, placing an order does not automatically create an account. Most customers will create their OneTest account as part of the registration process, after they receive their kits in the mail.

The 8 or 9-alphanumeric Control# is located on the top-middle of the requisition form.

Once you have completed the registration process, you will receive a confirmation email at your account email address.

Please enter your information accurately so we can contact you when your test result is ready.  After your complete registration, instructions will be sent by email for the next steps to follow for specimen collection and returning it to the lab.

Select whether you are using your own healthcare provider or our telemedicine provider for the medical authorization of the test.

Physician Information

OneTest requires authorization by a licensed medical provider.  You can use your own medical provider, or we can connect you with an independent telemedicine provider.  

If you do not know which medical provider you plan to use, you can leave this section blank.  However, when you send in your specimen, your test order form must have a medical provider's authorization.   Your Test order form is able to be printed at the end of this registration process.
